How do I get started?
Getting started is easy!
- Talk to Mount Sinai Services staff about carrier screening and learn more about your options. Call us at (866) 330-8797.
- Order your carrier screening sample collection kit online or pick up a kit from your healthcare provider.
- Complete the Informed Consent & Requisition Forms.
- Collect a cheek (buccal) swab and ship with our prepaid pre-addressed FedEx package.
All sample collection kits come with the necessary forms to successfully complete the screen. However, if additional forms are needed, you can download them here:
